→ NEW/NEVER SEEN PATIENT BRAND NEW? Choose ADULT - NEW PATIENT EXAM or PEDIATRIC - NEW PATIENT EXAM (children under 13). Complete emailed intake form prior to your scheduled visit.

→ RETURNING PATIENT Previous patient but it has been OVER 6 MONTHS SINCE LAST VISIT or SUSTAINED A NEW INJURY? A RE-EXAM is REQUIRED. Choose RE-EXAM and Chiropractic Spinal Adjustment regardless of age. Complete emailed update form prior to your scheduled visit.

→ CURRENT-EXISTING PATIENT For a REGULAR chiropractic visit, choose ADULT or PEDIATRIC (children under age 13) Chiropractic Spinal Adjustment or EXTENED (over 13 only)..

→ CURRENT-EXISTING PATIENT For an EXTENDED chiropractic visit, choose EXTENDED Chiropractic Spinal Adjustment and Muscle Work (over 13 only)

→ RED LIGHT THERAPY For a RED LIGHT THERAPY session, choose RED LIGHT THERAPY FIRST OR REGULAR depending on your status. (over 13 only)
